Roy, I agree with you about having one line in focus, but without using photoshop(as was asked in the guidelines) the only way I could think of accomplishing that would be a very narrow depth of field and laying the chart down on a flat surface, I think that would have taken away from being easily recognized as an eye chart, I am more than eager to hear and learn how this could have been done and keep it in perspective.
