Thanks to several people including S. Blaney and J. Reddoch, this orchid has been correctly identified as the above species, which is considered by NatureServe Canada to be "critically imperiled in Ontario". I had incorrectly identified it as Lesser purple fringed orchid, which you can see here: http://www.pbase.com/laroseforest/image/167100046
