Lensbaby 3G with no aperture stop inserted - full f2 aperture - shooting hand held by cabinet lighting in a jewellery store while getting a watch sent in for service. Through a glass cabinet, but it's the Lensbaby which adds the blur circles and strange FX. See next pic for a detailed section.
