I've newer understood this assertion.

Actually this shot is the painful result of an obstetrician (me) with a camera,
an anesthesiologist with no bashfulness and too much free time in L&D
and their boredom whileawaiting a recalcitrant fetus to negotiate its way
through a barely adequate pelvis.
