Here's what can happen when the fit of the coupling is too sloppy from a removal and re-installation.
Remember a straight coupling to the shaft is a light interference fit. Removing a rusted coupling, and breaking that rust layer free, can destroy this very, very narrow window of "fit".
Click this image and look at the key. It should NOT look like this. This is caused by a poor fit!
