There is no denying that "operator error" accounts for most of the quality issues here. But it should be pointed out that in situations like these it is quite common that the instant one's body language shows an intention to shoot someone, an unexpected little face shows up in the frame just as one is depressing the shutter button. They are quick those narcissistic rascals.
