The bright white specks/flare type objects in the photo appear to be dust particles in the air, reflecting the flash. The size and brightness of the flare specks are probably due to the distance from the flash, ie closer equals brighter. It seems to me that the problem appears when the flash is insufficient to properly expose the subject, because of distance or power or both. The image is underexposed, which worsens the problem.
