Wow, I can't believe I've never seen this rig, Tim! This is simply amazing. I knew that van was like something out of Inspector Gadget, but this takes the cake!

The ladder has been heavily modified and the legs are bolted to specially reinforced areas of the platform. Note the extra black-painted horizontal braces below the typical "fold-up" braces near the middle of the ladder.
The pole also screws into a specially reinforced area on the platform and is clamped to the extra horizontal brace on the right side and again at the top. The pole has a 1/4-20 stainless steel thread so a photographic ball mount can be installed although I use it most often as something to hold onto whilst shooting freehand.
The ladder-pole assembly also can be used as an antenna support for amateur radio emergency communications.
