This particular scene was staged by Ralph Lauren. The guy in the wheelchair is a mannequin.

I feel with some of the viewers of the image that it is sad because there is a strong element of truth conveyed about our social and human values.

Thanks for providing the background on the image. I'm understanding that Ralph Lauren staged this, and it sure is a powerful statement, and sort of a curious paradox. A couple of weeks ago a made a similar image (in Sreet of Dreams gallery), but not as direct and explicit as this one. Congratulations on your commitment to documenting the world near where you live. Not easy. But the results are worth it.
