First you shoot the images. They have to overlap about 30%.
Use manual settings (aperture, speed, White balance).
If you need only landscapes you can do it with out a tripod, otherwise for objects near you, must use a tripod with a panoramic head . (I Use Manfrottos 303 SPH)
How to stitch them.
You can use a program like photoshop or a more dedicated one.
I use PTAssembler. http://www.tawbaware.com You will find great info there .
Also I know of another more sophisticated program called Stitcher from REALVIZ but as far as I know is more expensive.
If you’re using a film camera you have to develop and scan the film first.
You have to ask to the photo lab not to apply different settings to each image but the same to all of them .
Do the same with scanning.
